Job Summary The Care Coordinator works closely with the Psychiatric Nurse Practitioner (APRN) and Psychiatrist to support individuals receiving outpatient medication management services. This role monitors medication effectiveness and side effects, coordinates care, and facilitates case and family conferences as needed. The program focuses on educating individuals, families, and caregivers on managing mental illness to promote independence and improve quality of life. Responsibilities Outpatient Services Completes initial and ongoing assessments, including substance use, living situation, support systems, mental status, history, strengths/needs, and medical status. Monitors and supports mental stabilization through regular contact and assessment. Coordinates services outlined in treatment plans, including referrals to community resources, and evaluates effectiveness and satisfaction. Maintains regular contact during medication management appointments and via phone as needed. Advocates for individuals and ensures rights are upheld in accordance with state and federal regulations. Assesses risk factors, trauma symptoms, and signs of abuse, neglect, or abandonment; reports concerns per protocol. Documentation and Reporting Maintains accurate, timely, and comprehensive documentation of all contacts, services, and communications. Develops and updates treatment plans and completes required reviews with individual participation. Monitors and documents progress toward treatment goals. Obtains and maintains appropriate consents and releases of information. Documentation Tool (Eleos Health) Reduces time spent on documentation, allowing more focus on client interaction and care. Transforms brief notes or session conversations into complete, clinically appropriate documentation. Supports accurate, compliant documentation while allowing staff to review and finalize notes. Provides helpful insights from sessions to support care planning and continuity. Accessible on desktop or mobile for flexibility across different work settings. Qualifications Bachelors degree in Social Work or a related Human Services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of course work includes the study of human behavior and/or development) required. Experience working in human services or mental health related field preferred. Must demonstrate proficiency in MHRC EHR System within three months of employment. Proficiency in Microsoft Office and email are required. Must be able to communicate and write English clearly, legibly and correctly at the college graduate level in order to make entries in the records of persons served. Documentation, paperwork and system entries must meet internal and external guidelines for content, accuracy and timeliness. Excellent customer service skills are essential and this individual must be able to interact appropriately with internal and external customers, including individuals served, families, caregivers, community service providers, supervisory staff and other department professionals. Position Details This position is a Full Time Days position: Monday through Friday, 8:00am to 4:30pm.
