The Family Advocate provides limited skills training and support services in the clients' home to help families work toward harmony; learn to support each other through everyday challenges and to reach everyday goals.

Location: This position is based at LifeWorks NW's St. John's site / Multnomah Co. Office

Pay/Benefits : $24.39-$31.71 per hour plus Full benefits.

Salary Placement Notice:
The posted salary range reflects the compensation potential for this role. Most candidates will be placed between the entry level and midpoint of the range, depending on their experience and qualifications. However, more seasoned professionals with extensive exact or similar experience may be considered for placement toward the higher end of the range. Final salary offers are made in alignment with internal compensation processes in accordance with Oregon Pay Equity laws.

Essential Responsibilities:

  • Works with families and service providers to minimize the potential for child abuse; makes home visits to families.
  • Provides skills training, parent education, and support services in the home. Plans and coordinates lessons for each session.
  • Help families recognize their personal strengths and develops a plan of action to address areas in which they want to improve. Consults with supervisor on cases involving potential domestic abuse or other family problems affecting clients' development.
  • Links families with community resources. Collaborates with community partners.
  • Uses program outcome measures as assigned.
  • Maintains chart notes related to external contracts, tracks client's involvement, follow-through and progress in program activities. Documents and maintains client records to ensure compliance with agency and contract guidelines and confidentiality. Completes documentation in an electronic health records system and scheduling program within agency specified timelines.
  • Completes critical incident reports when indicated within 24 hours of a critical incident.
  • Checks e-mail/voice mail daily/throughout the day when working responding to contacts within 48 hours for routine matters (urgent matters may require a more immediate response).
  • Travels between sites or in the community.
Qualifications:
  • Bachelor's degree in area of specialization directly related to work performed e.g., psychology, counseling, social work, etc. or a minimum of 3 years of prior experience with the related population.
  • Additional 2 years of specific experience providing skills training or related services to high-risk families.
  • Demonstrated ability to communicate effectively; understand mental health assessment, treatment and service terminology and individual, group, family and other counseling techniques; program policies and procedures for services and supports identified in an Individual Services and Supports Plan.
  • Demonstrated ability to provide culturally competent treatment services to diverse client populations and maintain a cooperative working relationship with others in a culturally diverse environment.
  • Demonstrated ability to provide case management services and compliance with confidentially in accordance with federal, state and/or funding source requirements.
  • Knowledge of community resources and treatment agencies enabling appropriate referrals for specific client needs.
  • Proficient in Microsoft Outlook and Word. Ability to master proprietary software including electronic health record system required.
  • Requires access to a car and a valid driver's license.
