Social Worker - Part Time, Emergency Department Join to apply for the Social Worker - Part Time, Emergency Department role at Upstate Medical University . Job Summary In this position, the Social Worker in the Emergency Department will provide comprehensive psycho-social assessment, management, and support to at-risk patients and families at Upstate Medical University Hospital. Knowledge of social determinants, population health, and drivers of utilization is essential. Duties include assessment, counseling, intervention, planning, and referrals to meet psycho-social needs. The candidate will learn to initiate and implement advance directives. The social worker will actively manage patients through collaboration, coordination, monitoring, evaluating options, and identifying services to meet individual health needs. They will round on assigned patients with the multidisciplinary team and participate in quality meetings within their unit. The role requires strong communication skills, knowledge of community resources, thorough documentation, participation in departmental meetings and quality activities, and other duties including coverage for colleagues and back-up responsibilities. Minimum Qualifications MSW from an accredited Social Work program (CSWE); M.S.W., L.M.S.W., L.C.S.W., or eligible; 3 years relevant post-master's experience (preferably in healthcare). New M.S.W. graduates who are L.M.S.W. eligible may be considered for an entry-level position after obtaining a Limited Permit, which must be converted to full licensure within 12 months. Preferred Qualifications Work schedule includes overnights (11pm-7:30am) with every other weekend rotation, and occasional on-call/holiday coverage based on operational needs. Additional Information This is a part-time position. The recruitment office is Human Resources.
