JOB DESCRIPTION

Kahak Health Care Services (KHCS) is seeking compassionate, responsible, and dedicated individuals to serve as Host Home Providers in a healthcare setting in NE Washington, DC. A Host Home Provider opens their personal residence to individuals with intellectual/developmental disabilities, mental health needs, or other healthcare-related conditions, offering a safe, nurturing, family-like environment. Under the direction of a registered nurse or case manager, the Host Home Provider supports clients' activities of daily living (ADLs), health management, and overall well-being while fostering independence, dignity, and community inclusion.

KEY RESPONSIBILITIES

  • Provide a safe, clean, and welcoming home environment for individuals placed in the Host Home program.
  • Assist clients with personal care tasks including bathing, grooming, dressing, toileting, and oral hygiene.
  • Support mobility, including transfers, ambulation, and the use of mobility aids or adaptive equipment.
  • Prepare nutritious meals, assist with feeding as needed, and monitor clients' nutritional intake per care plans.
  • Administer medication reminders and assist clients in following prescribed healthcare routines (as directed; not self-administered).
  • Monitor and promptly report changes in clients' health status, behavior, or living environment to the supervising nurse or care coordinator.
  • Provide meaningful companionship, emotional support, and social engagement to promote mental well-being.
  • Transport clients to medical appointments, therapy sessions, community activities, and other scheduled events.
  • Maintain a clean, organized, and hazard-free home environment, including light housekeeping duties.
  • Maintain accurate, timely documentation of care activities, incidents, and progress notes as required by KHCS.
  • Participate in required training, team meetings, and supervision sessions with KHCS staff.
  • Comply with all applicable local, federal, and agency regulations, policies, and client confidentiality requirements.

QUALIFICATIONS

  • High school diploma or equivalent required.
  • CPR and First Aid certification (must be current or obtained prior to placement).
  • At least 6 months to 1 year of experience in home care, residential care, or a related healthcare setting preferred.
  • Ability to read, write, communicate, and follow care plans in English.
  • Must reside in a home that meets program safety and habitability standards (home inspection required).
  • Physical ability to assist clients with mobility, transfers, and household duties.
  • Must pass a criminal background check, drug screening, TB test, and home safety inspection.
  • Valid driver's license, vehicle insurance, and access to reliable transportation required.
  • Must be at least 21 years of age (per DC residential care program requirements).

  • May involve occasional community outings, medical appointments, and transportation within the Washington, DC metropolitan area.
  • Possible exposure to household pets or allergens within the provider's home environment.

SCHEDULE & PLACEMENT OPTIONS

  • Full-time residential placement (client resides in provider's home).
  • Respite or short-term Host Home placements available.
  • Flexibility in number of clients hosted, subject to program capacity and home inspection approval.
